Any of the following:
[ ] Grade 10 Completer (with SF9/Form138/Latest Report Card)
[ ] PEPT Passer or A&E Passer (with Result of PEPT or A&E Test)
[ ] ALS Passer (with Certificate/Portfolio Assessment)

[ ] Basic Education Enrollment Form
[ ] Copy of PSA Birth Certificate or equivalent
[ ] Copy of Latest Report Card (SF9)
Important: Please make sure you have a scanned copy/picture
of the documents before proceeding with your early registration.
